Please
be advised that the undersigned, being all of the Trustees of the below named
Trust, request that the assets described below be transferred into the ownership
of the Trust, based on the following information:

The
asset should either be owned by the Trust or
the Trust should be named as the designated beneficiary, as the situation
indicates.
Generally,
bank accounts and other investment accounts should be owned by the Trustees in
the name of the Trust.
Life
Insurance Policies and certain annuities should name the Trust as primary beneficiary. However,
some annuities should name the Trust as a secondary beneficiary.
For
IRAs and 401k accounts (qualified plans) , the spouse would be named as the
primary beneficiary . The secondary
beneficiary generally would be the Trust for minor children.
If the children are not minors, then they should be named as secondary
beneficiaries.
